Introduction: Verteporfen is a second-generation porphyrin photosensitizer that can be activated by light (wavelength 689nm) and can be used in photodynamic therapy for macular degeneration. The drug selectively enters abnormal blood vessels, and through non-thermal laser irradiation, generates reactive oxygen species that block these vessels, thereby stopping vascular leakage and preserving vision. After photoactivation, verteporfen forms cytotoxic products in an aerobic environment. In this process, the porphyrin transfers absorbed energy to oxygen, forming highly reactive but short-lived monooxygen. Monooxygen destroys various biological structures within its diffusion range. This process leads to local vascular occlusion, cell damage, and, under certain conditions, cell death. In plasma, verteporfen is primarily transported by low-density lipoprotein (LDL). The use of verteporfen in photodynamic therapy (PDT) is selective. This is due to both the choice of light irradiation site and the enhanced expression of LDL receptors in rapidly proliferating cells, including choroidal neovascular endothelial cells, allowing these cells to selectively and rapidly take up and accumulate verteporfen. Choroidal neovascularization takes up verteporfen more rapidly and selectively than the retinal pigment epithelium, while the drug content is very low in other ocular structures. Therefore, it can selectively destroy choroidal neovascularization. It is used for diseases such as wet age-related macular degeneration, pathological myopia, and ocular histoplasmosis, which cause persistent vision deterioration or loss due to subfoveal choroidal neovascularization.
